Personal Data Collected prior to participating in a class at Good Life Yoga
A certain amount of personal data is collected by the teacher prior to class. This is for the purposes of meeting Insurance Company criteria as well as for your health and safety during yoga practice. Health information is also used to tailor sessions to suit your individual needs.
All information gathered is treated as confidential and will not be shared with a third party, unless it is in your best interest, i.e. a cover teacher is standing in and there is a significant health issue that they need to know about, for example if you are attending class while pregnant or undergoing treatment such as chemotherapy. The information is kept securely in a locked cupboard and only viewed by Alex Reed and the teacher of the class.
You may, of course, decline to submit personally identifiable information, in which case Good Life Yoga may not be able to provide certain services to you. If you wish to alter any of your data, review how it is used or wish to request its return to you, please contact Alex Reed, firstname.lastname@example.org, 07943 602607.
On the health form you fill in at Good Life Yoga, you are asked for your telephone number, this will only ever be used by the teacher for matters that personally relate to you, for example to inform you if there is a problem, they are running late or need to cancel/re-arrange class, or perhaps if you have left something at the studio.
Emergency Contact Information
Emergency Contact information is collected solely for use in an emergency, should Good Life Yoga need to contact your next of kin or give this information to the emergency services. This is the only information that would be shared with a third party, and only in the context of an emergency.
